This article examines the future of social security in emerging economies, exploring the key challenges facing these countries and proposing potential solutions. Financial sustainability, universal coverage and equity are some of the main obstacles that must be overcome to ensure a robust and fair social security system. Through a literature review and case analysis, proposals are presented to improve the effectiveness and sustainability of these systems in the context of emerging economies.

Psychological flexibility is a key concept of acceptation and commitment therapy. This factor has been linked with psychological wellbeing and associated factors, such as quality of life, in cancer patients. These and other positive results of acceptation and commitment therapy in cancer patients found in previous research could be enhanced by using mhealth tools. A three-arm randomized superiority clinical trial, with a pre-post-follow-up repeated measures intergroup design with a 1:1:1 allocation ratio is proposed. A hundred and twenty cancer patients will (...) be randomly assigned to one of the following interventions: face-to-face ACT + mobile application, face-to-face ACT, and Waitlist control group. The primary expected outcome is to observe significant improvements in psychological flexibility acceptance and action questionnaire- II in the face-to-face ACT + app group, after comparing baseline and post-treatment scores, and the scores will remain stable in the two assessment points, 3 and 6 months after the intervention. Secondary expected outcomes are significant increasing scores in quality of life and post-traumatic-growth, and significant decreasing scores in anxiety and depression, insomnia and fatigue at the same assessment points. Also, it is expected that the scores of this group will be higher than the scores of the face-to-face ACT group and the waitlist control group. This study aims to assess the efficacy of a combined intervention for psychological flexibility and associated symptoms in cancer patients. The results of this protocol may help to consider the use of acceptation and commitment therapy and mhealth applications in cancer settings as a valid therapeutic choice.Clinical Trial Registration[www.ClinicalTrials.gov], identifier [NCT05126823]. The concept of corporate social responsibility (CSR) is not new. Many entrepreneurs created and developed companies along the time, with a strong sense of ethical and social responsibility. This article presents an example of how CSR was conceived and put into practice when Caja de Pensiones para la Vejez y de Ahorros was created in Barcelona in 1905, following the life and ideas of its founder, Francesc Moragas, a lawyer with a deep commitment for social action and a successful conception (...) of the technical and economic dimensions of a financial and social institution. When Caja de Pensiones para la Vejez y de Ahorros, "la Caixa," was created in 1905, it was not only the transient response to a serious social, political, and economical problem, but also provided a permanent solution by creating a long-lasting social welfare institution. In addition, its founder understood the responsibility of social welfare institutions not as an isolated responsibility for each institution, but as part of a harmonious whole that is a real moral entity with a socio-economic character, with (...) autonomy and unity of nature, purpose, and form. The purpose of this article is to show how the corporate social responsibility conception of Francesc Moragas, the founder of "la Caixa," informed the activities of the new institution in a variety of dimensions, including its service to the customers, its economic strengths, the social work, and its participation in the creation of the public system of social security in Spain. Este artigo tem por objetivo apresentar e discutir a sugestão de Fred Dretske para analisar o problema dos qualia. Tal problema, caro à Filosofia da Mente, ficou conhecido pela discussão desenvolvida por Thomas Nagel em seu clássico artigo What is it like to be a bat. Nesse artigo, Nagel postulou a impossibilidade de se conhecer, em perspectiva de terceira-pessoa, os aspectos da experiência humana. Ele considera que, mesmo após as descrições objetivas da experiência de um sujeito, escapariam ainda aspectos qualitativos, (...) fundamentais para se caracterizar os qualia. A partir de sua Tese Representacionista da mente, Dretske argumenta que seria possível dissolver esse problema se admitirmos que a mente é a face representacional do cérebro, a natureza dos qualia seria, assim, representacional. Nesse contexto, os fatos mentais relacionados às experiências seriam fatos representacionais: se conhecermos a natureza desses fatos representacionais conheceremos também a experiência do sistema que a representa. Diante de tal entendimento, discutimos em que medida a proposta dretskeana constitui uma alternativa ao problema dos qualia. (shrink)

Background: The decision whether to initiate cardiopulmonary resuscitation may sometimes be ethically complex. While studies have addressed some of these issues, along with the role of nurses in cardiopulmonary resuscitation, most have not considered the importance of nurses acting as advocates for their patients with respect to cardiopulmonary resuscitation. Research objective: To explore what the nurse’s advocacy role is in cardiopulmonary resuscitation from the perspective of patients, relatives, and health professionals in the Basque Country (Spain). Research design: An exploratory critical (...) qualitative study was conducted from October 2015 to March 2016. Thematic analysis was used to analyse the data. Participants: Four discussion groups were held: one with patients and relatives (n = 8), two with nurses (n = 7 and n = 6, respectively), and one with physicians (n = 5). Ethical considerations: Approval was obtained from the Basque Country Clinical Research Ethics Committee. Findings: Three significant themes were identified: (a) accompanying patients during end of life in a context of medical dominance, (b) maintaining the pact of silence, and (c) yielding to legal uncertainty and concerns. Discussion: The values and beliefs of the actors involved, as well as pre-established social and institutional rules reduced nurses’ advocacy to that of intermediaries between the physician and the family within the hospital environment. On the contrary, in primary health care, nurses participated more actively within the interdisciplinary team. Conclusion: This study provides key information for the improvement and empowerment for ethical nursing practice in a cardiac arrest, and provides the perspective of patients and relatives, nurses and physicians. Our goal in this paper is to articulate a precise concept of at least a certain kind of disease-mongering, showing how pharmaceutical marketing can commercially exploit certain diseases when their best definition is given through the success of a treatment in a clinical trial. We distinguish two types of disease-mongering according to the way they exploit the definition of the trial population for marketing purposes. We argue that behind these two forms of disease-mongering there are two well-known problems in the (...) statistical methodology of clinical trials (the reference class problem and the distinction between statistical and clinical significance). COVID-19 confinement has supposed a challenge to the whole wide world, especially in athletes that have frustrated their expectations about training programs and competitions. Specifically, competition cyclists during confinement had lot of difficulties to train due to the need to train outside their homes. However, the increase of online training sessions, or virtual training tools could help to overcome training difficulties due to confinement although there are not studies that analyse the effects of using these types of tools in cyclists. This study aims to test how the use of online training tools in competition cyclists during confinement is related to training frequency and duration, and emotions. 329 cyclists filled an online questionnaire about feelings during confinement and the use of online training tools, before and during confinement. Results showed that the use of online training tools was associated with higher frequency and duration of trainings. Moreover, those who used these tools felt more positively about their future and preparation to compete, feeling more energy than those who did not used online tools. In addition, cyclists that started using these online tools (including virtual roller training) during confinement increased the frequency of training. In conclusion the use of online training tools for cycling could be appropriate to maintain training levels and energy and positive feelings about their future. (shrink)

RATIONALE AND AIMS: Total hip and knee replacements, usually, have long waiting lists. There are several prioritization tools for these kind of patients. A new tool should undergo a standardized validation process. The aim of the present study was to validate a new prioritization tool for primary hip and knee replacements. METHODS: We carried out a prospective study. Consecutive patients placed on the waiting list were eligible for the study. Patients included were mailed a questionnaire which included, among other questions, (...) the seven items of the priority tool and the Western Ontario and McMasters Universities Arthritis Index (WOMAC) specific questionnaire. The priority tool gives a score from 0 to 100 points, and three categories (urgent, preferent and ordinary). We studied the content and construct validity. We used Student's t-test or one-way analysis of variance. Correlational analysis was used to evaluate convergent and discriminate validity. RESULTS: The sample consisted of 838 patients (62.3% were female), with mean age of 70.2 years (SD 8.4). A total of 55.5% patients underwent knee replacement. Given that the tool was elaborated by patients and orthopaedic surgeons, it shows a good content validity. The priority score was statistically different (P < 0.001) among the three urgency categories created. The scores of the three WOMAC dimensions showed differences (P < 0.001) by the three urgency categories created. The correlations between the priority score and WOMAC dimensions were 0.79 (function), 0.69 (pain) and 0.51 (stiffness). The correlations between WOMAC items and items from priority tool were greater (0.47-0.69) between items measuring similar constructs than those measuring different constructs (0.27-0.49). These data are similar in both joints. CONCLUSIONS: Results support the validity of the prioritization tool to be used with patients waiting for hip or knee replacement. (shrink)

Background: Serious games have shown positive results in increasing motivation, adherence to treatment and strengthening the therapeutic alliance in multiple psychiatric disorders. In particular, patients with impulse control disorders and other disorders in which the patient suffers from inhibitory control deficits have been shown to benefit from serious games.Aim: The aim of this study was to describe the characteristics and to evaluate the usability of a new serious videogame, e-Estesia. This serious videogame was designed to improve emotion regulation in patients (...) with gambling disorder. Preliminary results from a pilot sample are also reported.Method: A pilot sample of 26 patients undergoing treatment for GD was recruited. Participants used e-Estesia on a tablet, which was connected to a thoracic band that sent heart rate and heart rate variability data to the videogame platform in order to provide biofeedback. The System Usability Scale was completed by patients to determine the usability of e-Estesia.Results and Discussion: e-Estesia performed comparatively well for all the explored groups. Around 84.6% of the patients endorsed that it was easy to use. Female patients with GD presented higher HRV during the use of the serious videogame compared to men. (shrink)

La indagación del segundo Heidegger acerca del ser encuentra su maduración en la doctrina del Ereignis, un nuevo concepto «post-metafísico», según Vattimo, con el que el filósofo alemán trata de responder a la pregunta de qué es el ser una vez ha mostrado, en su primera etapa intelectual, la esencial diferenciación ontológica. El ser no es un ente, ni aquello fijo e inmutable que fundamenta lo ente. Ni de él puede decirse que es; el ser «se da». Y se da (...) al hombre en un acto de donación recíproca en la totalidad del acontecer histórico. Vattimo parte de esta idea para concluir que el ser es evento que orbita en la constelación del Ge-Stell, el destino de la época actual en cuanto forma de desocultar el ser, de desvelar la verdad, y convierte a Heidegger en precursor de la postmodernidad y de su pensiero debole. Abordamos aquí unos aspectos, posiblemente los menos conocidos, de la dilatada vida de Fray Diego de Deza, como son los orígenes de su linaje en la gallega Terra de Deza y el afincamiento durante la segunda mitad del siglo XIV de la que fue la rama familiar de Toro, su ciudad natal. No perteneciendo a la hidalguía de mayor abolengo y riqueza de los linajes allí establecidos, sus méritos personales le labraron un futuro prometedor, primero por la cátedra docente que (...) ocupó en Salamanca y sus altas dotes como maestro y, además, por la confianza que los Reyes Católicos depositaron en él al nombrarlo preceptor del malogrado príncipe heredero don Juan, lo que le hizo emprender una envidiable carrera tanto eclesiástica como por sus servicios a la Corona. Nuestro objetivo aquí no ha sido otro que vislumbrar sus relaciones parentales y sociales, obviando su brillante cursus honorum en ese doble rol de servicio al altar y al trono. (shrink)

One of the main challenges in medicine is to guarantee an appropriate drug supply according to the real needs of patients. Closed-loop strategies have been widely used to develop automatic solutions based on feedback variables. However, when the variable of interest cannot be directly measured or there is a lack of knowledge behind the process, it turns into a difficult issue to solve. In this research, a novel algorithm to approach this problem is presented. The main objective of this study (...) is to provide a new general algorithm capable of determining the influence of a certain clinical variable in the decision making process for drug supply and then defining an automatic system able to guide the process considering this information. Thus, this new technique will provide a way to validate a given physiological signal as a feedback variable for drug titration. In addition, the result of the algorithm in terms of fuzzy rules and membership functions will define a fuzzy-based decision system for the drug delivery process. The method proposed is based on a Fuzzy Inference System whose structure is obtained through a decision tree algorithm. A four-step methodology is then developed: data collection, preprocessing, Fuzzy Inference System generation, and the validation of results. To test this methodology, the analgesia control scenario was analysed. Specifically, the viability of the Analgesia Nociception Index as a guiding variable for the analgesic process during surgical interventions was studied. Certain dialogism is generally recognized in the founding moments of Western philosophy, and it is possible to see various forms of intertextuality in our philosophical tradition. In spite of certain monological temptations, philosophy is not a self-demonstrative system; it has woven its texts quoting other discursive voices and through the invocation of borrowed words. This article aims to outline a discontinuous overview of quotation rhetorical functions at different moments of Western philosophy. The philosophical quote has contributed to discursive authorization or (...) to provide shared discursive repertoires, but also to invocate other intertextual voices discursive or integrate other texts in the meta-discourse of the philosopher. Nowadays the disciplinary regulation of citation practices and the bibliometric economy of impact indices threaten that dialogic and intertextual legacy. (shrink)

The objective of this article is to analyze the production of the subject Woman by reviewing some practices, discourses, and technologies promoted by the state, the church, and elites. It is important to emphasize that in most research about women or femininity, female subjectivity appears tightly linked to sexual difference. However, in this work I want to show that the notion of Woman is co-determined by race and class. The experience characteristic of such representation was possible only for a small (...) group of white and bourgeois women. Others—Indian, black, and mestiza “women”—could hardly account for a social experience comparable to the Western narrative about woman. Nevertheless, processes of homogenization allow these others to be classified and disciplined according to the gender norm, yet without altering the prejudices and inequalities produced by the prevailing racist and classist system, which implies the production of other female subjectivities, of other “women.”. El confinamiento vivido durante la pandemia de Covid 19 nos obligó a afrontar la vida con un ritmo más tranquilo. En este contexto, desde el Grupo de Estudios Interdisciplinares de Literatura y Arte (LyA) (UCLM) se planteó desarrollar un proyecto donde abordar nuestra docencia desde una perspectiva más pausada siguiendo la filosofía del Slow Movement.
